The `ResultConverter::convertStream()` method only handled text content deltas, ignoring
`tool_use` blocks entirely. This caused tool calls to be lost when streaming was enabled.
This fix properly handles Anthropic's streaming format for tool calls:
- `content_block_start` with `type: tool_use`
- `content_block_delta` with `input_json_delta`
- `content_block_stop` to finalize tool calls
- `message_stop` to yield the `ToolCallResult`
This is the Anthropic equivalent to #1107 (Gemini).
