Hannes Magnusson 97d53b218c Upgrade to DocBook5: ...

- All id attributes are now xml:id
 - Add docbook namespace to all root elements
 - Replace <ulink /> with <link xlink:href />
 - Minor markup fixes here and there
 - Bump EN-Revision where appropriate
