Simon Roberts 8c04732cb2 Changes to make annotations work on mirrors, by storing the comments in 
manual/usernotes. Just open
  http://your-mirror/get-user-notes.php
then
  chmod -w /MIRROR/manual/usernotes
to stop someone else doing it again (generates ~2Mb of traffic to php.net)
