Update the Changelog to show progress of 4.0 from beginning.

This is just an HTML-ization of the NEWS file from the latest
distribution. I notice it's missing some info (i.e. changes
that went into 4.0.3pl1), so anyone can feel free to update it.
And we should update it on each release. :)
