Craig Francis 50eca61f68 Use ENT_QUOTES|ENT_SUBSTITUTE default for HTML encoding and decoding functions 
htmlspecialchars() etc now use ENT_QUOTES | ENT_SUBSTITUTE rather
than ENT_COMPAT by default.

Closes GH-6583.
