Migrate more SKIPIF -> EXTENSIONS (#7139)
This is a mix of more automated and manual migration. It should remove all applicable extension_loaded() checks outside of skipif.inc files.

PDO: Honor ATTR_STRINGIFY_FETCHES for booleans
Of the important PDO drivers, this affects only PDO PgSQL, as
both MySQL and SQLite do not return native boolean types.
