These PHP3_* macros aren't defined in the code, so these haven't been doing
...
anything for some time now (at least since 4.0 was released). So let's go
ahead and remove those.

Added a few RCS $Id$ tags.

Clean up php3.*\.h files. The files itself are renamed, and references in all

.*\.[ch] files were changed. There is a slight chance that my script missed
a few changes, please correct them manually.
