Migrate more SKIPIF -> EXTENSIONS ( #7139 )

This is a mix of more automated and manual migration. It should remove all applicable extension_loaded() checks outside of skipif.inc files.

Fix internal_encoding fallback in mbstring
...
By introducing a hook that is called whenever one of
internal_encoding / input_encoding / output_encoding changes, so
that mbstring can adjust it's internal state.
This also makes internal_encoding work with zend multibyte.
