Clean DONE tags from tests
Remove most of the `===DONE===` tags and its variations.
Keep `===DONE===` if the test output otherwise becomes empty.
Closes GH-4872.

Fixed bug #44995 (bcpowmod() fails if scale != 0)
`bc_divmod()` is supposed to do integer division, so we must not apply
a scale factor here.
