Replace n/a and invalid EN-Revision hashes with the oldest commit
hash from doc-en. This moves these files from "missing tags" to
"outdated" in revcheck, making translation status more actionable.
- All id attributes are now xml:id
- Add docbook namespace to all root elements
- Replace <ulink /> with <link xlink:href />
- Minor markup fixes here and there
- Bump EN-Revision where appropriate
