From now on, manual.ced will need to be found at ~/.phpdoc/manual.ced.

- (Created missing setup sections in setup.xml, if any)
- Moved the intro to book.xml
- Changed the intro ID from <extname>.intro to intro.<extname>
- Created empty constants appendix (constants.xml)
- Moved the examples into its own chapter (examples.xml)
NOTE: The chapter uses the ID 'java.examples' while the first example was renamed to 'java.examples-basic'
- Moved the requirements and resources sections to setup.xml
- Moved the configure and ini entities to setup.xml
- Moved the java.servlet section into its own chapter (servlet.xml)
