- Added libxml requirement to extensions requiring it (w/ new entity)

From now on, manual.ced will need to be found at ~/.phpdoc/manual.ced.

- All id attributes are now xml:id
- Add docbook namespace to all root elements
- Replace <ulink /> with <link xlink:href />
