From now on, manual.ced will need to be found at ~/.phpdoc/manual.ced.

- (Created missing setup sections in setup.xml, if any)
- Moved the intro to book.xml
- Changed the intro ID from <extname>.intro to intro.<extname>
- Moved the empty constants section to constants.xml and changed it to appendix
- Added constants entity to book.xml
- Moved the requirements, installation and resources sections to setup.xml
- Moved the ini entity to setup.xml
