af4410a7e1 Normalized the sgml-default-dtd-file local-variable line for those 
still using this, after discussion on the phpdoc list.
From now on, manual.ced will need to be found at ~/.phpdoc/manual.ced.

128868893b MFB: Upgrade to the new-reference-structure 
- Moved the intro to book.xml from reference.xml
 - Changed the intro ID from <extname>.intro to intro.<extname>
 - Moved the constants entity to book.xml
 - Changed constants.xml to be an appendix
 - Moved the examples from reference.xml to its own chapter in examples.xml
	NOTE: The chapter ID is fdf.examples, the example itself doesnt have an ID
 - Moved the requirements/configuration/resources sections to setup.xml
 - Moved the configure entity to setup.xml

577d06440a Removed information about copying files into the Windows system directory and 
instead discussed our preferred method: Putting the PHP directory in PATH.
# At some point have other extensions use the new &ext.windows.path.dll; too
