From now on, manual.ced will need to be found at ~/.phpdoc/manual.ced.

- The intro text (partintro)
- Moved from reference.xml to setup.xml:
- The requirements, installation, and other setup entities
- Changed constants.xml and examples.xml to be appendices
- Changed the intro ID from <extname>.intro to intro.<extname>
