Torben Wilson af4410a7e1 Normalized the sgml-default-dtd-file local-variable line for those 
still using this, after discussion on the phpdoc list.
From now on, manual.ced will need to be found at ~/.phpdoc/manual.ced.

Hannes Magnusson 769f449024 MFB: Upgrade to the new-reference-structure 
- (Created missing setup sections in setup.xml, if any)
 - Moved the intro to book.xml
	NOTE: Changed the mbstring linkend from ref.mbstring to book.mbstring
 - Changed the intro ID from <extname>.intro to intro.<extname>
 - Moved the constants entity to book.xml
 - Changed constants.xml to be an appendix
 - Moved the ini & configure entities to setup.xml
