where cache set up is happening, there's a requirement that is not addressed. If you follow the instructions you will get the following error: http://pastie.org/1276600.
The fix is this (from sandbox):

  1. Create a folder named Entities under APPATH.'models'
  2. add the following lines after "$config->setMetadataCacheImpl($cache);":
    $driverImpl = $config->newDefaultAnnotationDriver(array(APPPATH.'models/Entities'));
    $config->setMetadataDriverImpl($driverImpl);
