This causes compile failures on PHP < 7.3 that I have yet to understand.

executeBulkWrite() will now throw a BulkWriteException on top of any previous exception to ensure that a WriteResult can be attached. InvalidArgumentException is excluded, since that is only thrown before mongoc_bulk_operation_execute does IO (e.g. batch is empty).
In libmongoc 1.6.0, insert documents are validated during BulkWrite::insert(). This is a fragile test that fails when v1.2 is compiled against libmongoc 1.6.0 as a system library.
This changes WriteResult to encapsulate a bson_t, which is populated by mongoc_bulk_operation_execute(), instead of the private mongoc_write_result_t struct. This entailed significant changes to the WriteResult debug handler, so new tests have been added for it.
With phongo_execute_write() modified to populate a bson_t reply, we also changed error handling to pull a BulkWriteException message from bson_error_t (PHPC-626). That change required several error tests to be modified for the new message format.
