Hannes Magnusson 069f6629d8 - Bunch of build fixes ...

- Upgrade to DocBook5:
  - All id attributes are now xml:id
  - Add docbook namespace to all root elements
  - Replace <ulink /> with <link xlink:href />
  - Minor markup fixes here and there
