From now on, manual.ced will need to be found at ~/.phpdoc/manual.ced.

- (Created missing setup sections in setup.xml, if any)
- Moved the intro (the giant table) to book.xml
- Changed the intro ID from <extname>.intro to intro.<extname>
- Moved the constants entity to book.xml
- Changed constants.xml to be an appendix
- Moved the requirements and resources sections to setup.xml
- Moved the configure (installation section) and ini entities to setup.xml
NOTE: Also fixed the linkend in functions/filter-var-array.xml & functions/filter-input-array.xml
to point to the constants section
