f2f5c11726 Update debugger appendix (#2582) 
Remove links to Zend IDE (no longer available), dbg (supports until PHP 5.5) and apd (no releases since 2004).

This page could really be flashed out with how step debuggers like Xdebug are the way to comprehensively debug applications, but I don't think it's my place to write that documentation.

af4410a7e1 Normalized the sgml-default-dtd-file local-variable line for those 
still using this, after discussion on the phpdoc list.
From now on, manual.ced will need to be found at ~/.phpdoc/manual.ced.

d522d135a1 Removed PHP 3 information from the manual. Goodbye PHP 3, thanks for all the memories! 
This commit covers many files.
The CVS file phpweb/manual/php3.php will exist to store [most of] this info.

c030e2adf7 Upgrade to DocBook5: 
- All id attributes are now xml:id
 - Add docbook namespace to all root elements
 - Replace <ulink /> with <link xlink:href />
 - Minor markup fixes here and there

91e836093c - Standardize the Unix acronym. Unix is the preferred capitalization 
as UNIX is a registered trademark by the Open Group. References to UNIX
  were left in where it made sense, like example outputs, and so forth.

26bc4474fe Adding PHP 4 debugging links to debugger.xml and global.ent

6ae3b9dba5 Fix modelines for VI(M) for the manual (en). 
Syntax for vim < 6 is set to sgml, because xml is not recognizing tag-names.

This was discussed before, and this turned out to be best for all vi(m)
versions.
